The Chapter Achievement Awards were created to highlight the achievements of chapters in the area of Education, Organization, Communication and Community.
The application is a two step process: complete the online application; then make sure to click on the UPLOAD AWARDS DOCUMENTS tab at the bottom of the page to upload all support documentation.
But before you get started, please read carefully through the instructions below. Incomplete applications will not be accepted.
How to Apply:

Make sure to select one award category from the drop down menu.

You can change the award category at any point up until the submission deadline on January 11, 2010 at 5pm ET.

Before submitting your application, please review your supporting documentation to make sure it coincides with the selected award.

All answers must be completed on the online submission form.

Chapters applying for a Chapter Achievement Award can not apply for the Chapter Excellence Award.
Submission Requirements:

All entries must reflect activities from January 1, 2009 through December 31, 2009. Activities prior or subsequent to these dates should NOT be included, and will not be considered.

Submissions should represent activities that are portable from chapter to chapter. The application should highlight best practices that can be shared and may be duplicated among other chapters.

To prevent losing your answers due to technical/computer problems, it is recommended that you prepare and save a draft of your submission in a word processing document before entering the information online. Once you’ve completed your application, copy and paste the answers onto the online submission form.

By completing an award application, you agree to the submission becoming the property of RIMS, and that RIMS may post and/or publicize it in any media it determines, in its discretion, to be appropriate.

Multiple award winners are anticipated and encouraged! Chapters are not competing against other chapters. Awards will be given based on individual chapter achievement - measured against a specific award point threshold.
Supporting Documentation:

Supporting documentation must be submitted as one (1) .pdf file, no larger than 1MB.
Tip: Color documents tend to be larger in size then black & white documents.

Supporting documentation should be well organized, titled and numbered accordingly.

Submissions that are not well documented may result in penalized grading.

You must upload your documentation before submitting your application.

Once you submit your application you will no longer be able to make any changes to the support documents.
Submission Confirmation:
- RIMS will send an email by January 13, 2010 confirming your submission and supporting documentation have been received.
Notification:
- Notification of winners will be made to the Chapter's Board of Directors in care of the Chapter President.
Judging:
- Your submission must receive a minimum cumulative score of 75 to be considered for the Chapter Achievement Award.

20 points Identify, evaluate and quantify success
20 points Identify ability to recreate elsewhere
